Verruciform xanthoma is a lesion that can occur in the skin, oral, and genital mucosa. Its etiology is unknown; however, it appears to be an unusual response to epithelial damage or trauma. Its clinical appearance is similar to an inflammatory hyperplasia. This report describes the case of a 26-year-old man with a lesion in the vestibular gingiva of tooth 18, which was initially associated with biofilm accumulation and local trauma. The presence of an asymptomatic elevated reddish nodule of soft consistency and measuring 1 cm in length was observed. In this case, an excisional biopsy was performed, and the histopathological examination revealed a papillary and acanthomatous epithelium, with the presence of xanthoma cells confined to the papillae of the connective tissue. The final diagnosis was verruciform xanthoma, the treatment being the total removal of the lesion.
