Mandibular osteotomies have wide indication in the correction of dentomaxillofacial deformities. Puricelli, in 2007, proposes a concept and new technique of mandibular osteotomy. A medial osteotomy of the mandibular ramus is performed, above the lingula. The interdental mesial space of the first lower molar is adopted as a reference for the vestibular trait, in the vertical direction. This advancement of the osteotomy allows an approximate anteriorization of up to 20 mm.
